Dear Parents:
For the school year 2010-11, SS. Peter and Paul will be offering volleyball, basketball and track and field for boys and girls in grades 5 through 8. The fall sports will be girls’ basketball and boys’ volleyball, followed by boys’ basketball and girls’ volleyball in the winter. Track and field will be offered for both boys and girls in the spring.
In the attached packet, you will find all the information you will need to complete in order for your child to be eligible for sports at SS. Peter & Paul next year.
PHYSICAL FORM. Please note this is a new form. Diocesan policy states that current physicals must be on file at the school before a child is able to begin PRACTICE. Physicals are valid for one calendar year. The first season’s practice will begin on Wednesday, August 18th . Please take care of physicals BEFORE this time. Any missed practices will put your child at an immediate disadvantage. The last day to turn in physicals and begin practice in the fall will be Wednesday, August 18th. If your child’s physical is not turned in by 3:00 PM that day, he/she will be unable to join the first sport season.EMERGENCY
TREATMENT FORM, a two-sided form. This is similar to the form you complete for field trips. We keep an additional form for those involved in sports.
UNIFORM ORDER FORM. We will continue to use the same uniforms next year. Our uniform includes the ash gray, numbered T-shirt and solid navy shorts. It may be worn for all sports. All shirts must be purchased from school. You may purchase shorts from school, or provide your own. If you provide your own shorts, they MUST be solid navy, no brand logo, no school name, no stripes, NO EXCEPTIONS.
ACADEMIC ELIGIBILITY REQUIREMENTS. This will answer any questions you may have about the academic standards your child must maintain in order to be able to participate in sports.
SPORTS FEE/REGISTRATION FORM. A Sports Fee of $81.00 will be charged per sport. One dollar of this fee goes to Salpointe C.H.S. for the use of their facilities. If this fee is a hardship for your family, there is scholarship money available. Please see me if you are interested in this assistance.
DIOCESAN CODE OF CONDUCT. This form should be read and discussed by parent and child. Both sign the form as well.
FIELD TRIP FORM. All students participating in school-related activities, including sports, must have this form signed by a parent.
I will be available on “Pay Day” to collect all completed forms. Please have all forms completely filled out. Taking care of these matters will insure that your child will be able to begin with the rest of the team. Thank you for your help.
