Welcome to SS. Peter and Paul School,

Below you will links to the materials needed to register your child(ren) for SS. Peter and Paul School . Should you have any questions, please contact the School Office (325-2431) and we will be happy to assist you.
Please review the materials carefully and fill out EACH FORM COMPLETELY. A packet is needed for each child and it is imperative that we have this documentation.

INCOMPLETE FORMS WILL NOT BE ACCEPTED AND THE ENTIRE PACKET OF INFORMATION WILL BE RETURNED UNTIL ALL IS COMPLETED.

	____ Tuition/Policy-Procedure Commitment
	____ Emergency Information & Immunization Form – Blue Card Emergency Information & Immunization Record Form must be filled out completely for each child. All address and phone number information must be completed on form. Hospital and Doctors must have an address and phone number.
	____ Release of Information – Provides us with permission to include your child(ren) in SS. Peter and Paul publications including yearbook, website, newspaper or other printed media.
	____ Computer Information & User Agreement
	____ Affidavit of Intent for Private School
	____ Custodial Agreements (if any)
	____ Non-Refundable Registration Fee

Please submit with your packet copies of:

	Birth Certificate
	Current Immunization Record
	Current Physical
	Sacrament Certificates ( Baptism, Communion, Confirmation)
	Copy of Report Card from previous year or current one available
	Copy of Standardized Test Results
